Objective:To evaluate the efficacy and safety of a clinically tested novel, single dose of allopurinol in the treatment of obese and overweight patients with obesity and/or type 2 diabetes mellitus.
Materials and methods:This double-blind, randomized, placebo-controlled trial was conducted in two centers in the United States. A total of 673 patients (n=742) met the criteria for obesity (body mass index [BMI] >30 kg/m2), and were assigned to two groups: one group was given a placebo (n=106) and the other one was a group receiving allopurinol (n=98). A total of 103 patients (n=102) were randomly assigned (62 male, 66 female) to one of the groups, and the patients were then assessed for the presence of hyperlipidemia, hypertriglyceridemia, and/or hypercholesterolemia. Patients in the placebo group were treated with allopurinol for three months and were also treated with metformin orlistat for six months. Patients in the allopurinol group were treated with allopurinol for three months and were also treated with metformin orlistat for six months. All patients received at least one additional dose of allopurinol, except for the patients in the placebo group, and the treatment was continued until the end of the trial. Patients were instructed to continue the treatment for at least 12 weeks and to abstain from allopurinol for one week before starting treatment. Patients were instructed to abstain from metformin for 12 weeks or allopurinol for six months. All patients were required to be on an average of three weeks of diet and exercise for the entire three-month treatment period. Patients were instructed to abstain from allopurinol for one week after the first dose of allopurinol, and were also instructed to abstain from metformin for one week after the second dose. All patients who were unable to adhere to the protocol were also instructed to stop allopurinol for one week prior to the second dose. The study was performed in compliance with all pertinent guidelines and regulations, including the Declaration of Helsinki. Patients were randomized into two groups, one group received the allopurinol and the other was given the metformin.The subjects in the allopurinol and metformin groups were randomly assigned to one of the study groups. The secondary endpoint was the number of patients who remained obese and overweight during the entire three-month treatment period. Patients who were compliant with all treatment guidelines were also instructed to continue their treatment for one week prior to the first dose of allopurinol. The study was performed in compliance with the principles of the Declaration of Helsinki.
Statistical analysis was performed using the SPSS Statistics version 16.0 (SPSS Inc., Chicago, IL, United States) and the results were expressed as mean ± standard deviation or number (%).
Results:Among the 103 patients who were randomized, 6 (4.8%) in the allopurinol group and (10.8%) in the metformin group, and (10.8%) in the placebo group, were treated with allopurinol for three months. In the three-month treatment period, 6.5% of the patients in the allopurinol group and (11.3%) of the patients in the metformin group were compliant with their treatment guidelines, while 11.3% of the patients in the placebo group were compliant with their treatment guidelines. In the three-month treatment period, the mean weight loss was 2.7 ± 0.9 kg in the allopurinol group and 2.8 ± 1.5 kg in the metformin group (P = 0.038). The number of patients who were compliant with the treatment guidelines was 12.
